1. Application for admission to Arapahoe Community College
2. Copies of all college transcripts containing coursework applicable to PTA degree at ACC.
3. Letter of recommendation from your previous program director, commanding officer or
clinical supervisor.
4. Copies of previous program curriculum including course descriptions and syllabi of
completed courses.*
5. Documentation of honorable discharge from your branch of service, if applicable.**
6. Copy of your diploma from Ft. Sam Houston.**
